Referees

Whenever a Referee cautions or dismisses a player from the Field of Play or is reporting other forms of misconduct, he or she shall prepare a report of the incident or incidents. Within 2 days of the match (Sunday not included), this report must be submitted over the internet or sent by first class mail addressed to the Secretary of the appropriate Association or such person as is nominated by such Association. All such reports shall be made on the standard Misconduct Report Form as approved by the Council (see below) and as may be amended from time to time.

Assistant Referees

Neutral Assistant Referees shall, where the referee has dismissed a player from the Field of Play or is reporting other forms of misconduct, prepare and submit a report in a similar fashion as the match referee. A record should be kept of cautions issued in case the Association requires the Assistant Referee to submit a report.

Personal Hearings

Referees and Neutral Assistant Referees must attend Personal Hearings when required so to do. If a Referee or Assistant Referee fails to attend he shall be reported to the appropriate authority. Match Official appointments at Contributory League Level or above, already received, take precedence over requests to attend Personal Hearings. However, any Referee having accepted a request to attend a Personal Hearing is then required to close the date with all appropriate Competitions.

Online misconduct forms

The Football Association has introduced new online misconduct forms to assist Referees and the general administration of discipline. Previous styles of Report Form can still be used.

Caution Misconduct Report Form

Please ensure the instructions are followed and the offence (C1 to C7) is inserted. If you are reporting a C1 offence, please ensure you complete the last column as well to define the sub-category of offence. (FA reports only)

Standard Misconduct Form 

For reporting dismissals and other misconduct. Also to be used to report a player receiving two cautions in a match. Please indicate if you have forwarded a copy of the form to any other County.

To assist in the administration of the misconduct, please include team details on the form. (i.e. 1st, Reserves, U18, U17 etc).Please note that only the approved templates may be used by referees and not any personally designed electronic forms. The Completed Forms should be returned toDiscipline@HampshireFA.com or a single copy can be printed off and posted to County Office.
